The Leapfrog Group announced they have published the results of their Spring 2025 Hospital Safety Grade. Among the scores, 346 hospitals have earned “A” grades for at least five grading cycles, according to a release.

The Leapfrog Group has named these awardees "Straight A" hospitals. 11 of them have earned “A’s” for 13 years.

Additional notable findings:

  • 12 states (Alaska, Delaware, Hawaii, Iowa, North Dakota, Nebraska, New Mexico, Nevada, South Dakota, Vermont, West Virginia, and Wyoming) have had no Straight A hospitals in at least two years.
  • Utah, Rhode Island, New Jersey Connecticut, South Carolina and Virginia had the safest hospitals in 2025.
  • “Utah ranks #1 for the fourth consecutive Safety Grade round, and ties for #1 among states with the highest percentage of Straight A hospitals.”
